NDIS clients will be charged for the service appointment:
- Allied Health Assistance and/ or Support Work
- Travel charge (time)
- Travel charge (km’s)

Referral form
If you have been referred to Thriving Health by an organisation or will be self referring, there's a link below to the referral form. Once you click on the link, you can fill it out online and save it to your computer.
